General Booking Form.
Please complete the Course Booking Form for all courses and return with your deposit in order to secure a place on the course.
Which other Forms to complete:-
If you intend to start a full Open Water Course (or a Referral Course), please complete all sections of the following Forms
If you are starting a try dive ‘Discover Scuba Course’ the above 3 Forms are not required. Instead, please complete all sections of Form Discover Scuba Statement - PADI 10279 UK However, if the Medical section of the Form results in a ‘Yes’ answer, a further Medical Form (Medical Statement - PADI 10063 ) will also be required.
Please note:- a ‘Yes’ answer on any of the Medical Forms does not automatically bar participation, but usually just requires further medical background information from your doctor prior to any ‘in water’ activities; also, each medical question must be answered either yes or no (ticks or crosses cannot be accepted).
For participants under 18:-
In addition to the above (which need to be signed by both the participant and their parent/guardian) an Emergency Treatment Consent Form - Under18s - PADI 10088 UK must be completed.
